What this is
A free search over public exclusion and debarment lists published by the U.S. Department of Health and Human Services Office of Inspector General, the General Services Administration, and state Medicaid agencies. Every record is republished verbatim from its source, with a link to that source and the date it was retrieved.
A match here is not an identification
The bulk files these records come from are published without Social Security Numbers – OIG removes them under the Privacy Act. A matching name therefore cannot establish that a listed party is the person you are looking for, however exact the spelling. HHS OIG operates a verification tool that confirms identity by SSN or EIN. That step is the only one that closes the gap, and nothing on this site substitutes for it.
No result is not a clearance
Not every jurisdiction is covered. Some publish no list at all, some publish only PDFs or portals, and some block automated access. The coverage page states exactly which is which, and it is part of the answer to any search that returns nothing.
Not a consumer report
This is not a consumer reporting agency, a background check, or a screening service of record, and it is not offered for use in evaluating anyone for employment, credit, insurance, housing, or any other purpose covered by the Fair Credit Reporting Act. Hiring, contracting and credentialing decisions carry obligations this tool does not discharge. Use it to find and read the primary record, then act on the primary record.
Accuracy and corrections
Records are refreshed on a weekly pass and each one shows when its source was last successfully fetched. Where a source publishes a reinstatement, it is shown; where a source signals reinstatement by removing a row, the record is retained and marked as no longer listed rather than deleted, so an earlier result corrects itself instead of silently disappearing. If you believe something here is wrong, the corrections page explains what can and cannot be changed here, and what has to be corrected at the agency.
Use of the data
The underlying records are public government data. This service adds aggregation and provenance, not authority. It is free to use, including commercially, subject to a reasonable request rate; the API is rate limited per address. It is provided as is, without warranty of any kind.
