Under 42 CFR 455.436, a state Medicaid agency must confirm the identity and exclusion status of providers and any person with an ownership or control interest – on enrolment and monthly thereafter – against the HHS OIG LEIE and the SAM.gov exclusions list. Checking a state list alone does not satisfy it, and checking the LEIE alone does not either.

That is why a search here covers both federal lists and every state list that exists. It is also why results distinguish health care exclusions from general federal debarments: SAM.gov carries every agency’s actions with no health care connection, and treating one of those as an exclusion from Medicare would be wrong about a named person.

Every result here is a potential name match, never a confirmed identity. Federal and state exclusion files carry no Social Security Numbers, so a matching name cannot establish that a listed party is the person you are looking for. Confirm any match against the official source above and verify identity with HHS OIG before acting. Finding no match is not a clearance.
